The Zollinger-Ellison syndrome and ectopic gastric mucosa.

چکیده
The Zollinger-Ellison syndrome may be treated by removal of the tumour(s), total gastrectomy (Zollinger and Ellison, 1955), an H2 inhibitor, such as cimetidine (McCarthy, 1976) or a combination of these three methods. Our approach (Stadil and Stage, 1978; Welbourn and Galland, 1982) is to control the ulceration with cimetidine and then, if possible, to localize and remove the tumour. If this is not possible, we either continue with cimetidine or undertake a total gastrectomy, the choice depending upon factors peculiar to each patient. The aim of total gastrectomy is to remove the 'target organ' for the action of gastrin. The presence of a second, undetected, target organ may lead to unexpected postoperative problems.

Significance of gastric analysis with particular reference to the diagnosis of Zollinger-Ellison syndrome.
Various criteria in gastric analysis currently used for the diagnosis of the Zollinger-Ellison syndrome were assessed in cases of endoscopically normal gastric mucosa, gastric ulcer and duodenal ulcer. New criteria consisting of BSV greater than 350 ml/hr, BAO greater than 25 mEq/hr, MAO greater than 60 mEq/hr, and BAO/MAO greater than 0.7 were proposed to differentiate the Zollinger-Ellison sy...
متن کاملZollinger-Ellison Syndrome — Presentation, Diagnosis, Therapy
Zollinger-Ellison syndrome results from a pancreatic tumor in the non-beta islet cells, which stimulates the maximal activity of the gastric acid-secreting cells, resulting in gastrointestinal ulceration of the mucosa. Multiple endocrine neoplasia type 1 (MEN 1) is a familial autosomal dominant syndrome that includes Zollinger-Ellison syndrome as part of it; however, Zollinger-Ellison syndrome ...
متن کاملIsolation of two minigastrins from Zollinger-Ellison tumour tissue.
A pair of gastrin tridecapeptides (;minigastrins') have been isolated from Zollinger-Ellison tumour tissue; they correspond to the fragment 5-17 of the heptadecapeptides isolated from the same source. In one (type II) the tyrosine residue present is sulphated, in the other (type I) it is not. The proportion of types I and II is approximately 2:1 similar to that for the ;big' gastrins and the he...
متن کاملDuodenal Bulb Mucosa with Hypertrophic Gastric Oxyntic Heterotopia in Patients with Zollinger Ellison Syndrome
Objectives. Zollinger-Ellison Syndrome (ZES) results in hypersecretion of gastric acid (via gastrinoma) leading to peptic ulcers, diarrhea, and abdominal pain. We describe the novel discovery of hypertrophic, heterotopic gastric mucosa in the proximal duodenal bulb in patients with ZES, which we hypothesize results in an increased incidence of postbulbar ulcers in patients with ZES (a mechanism...

A B S T R A C T Gastric emptying and secretion, as well as intragastric volume and composition, were determined simultaneously in three patients with Zollinger-Ellison syndrome and in seven normal subjects. Gastric hypersecretion was observed in patients with ZollingerEllison syndrome and in normal subjects receiving pentagastrin. In contrast, the fraction of gastric contents emptied per minute...
